Short and to the point

Include name of the LGU

2. Summary

                        Gives overview of the project (300 words or ½ page)

Describe project and LGU where it was implemented

Explain strategy, methodology and stakeholders involved

Identify major accomplishments and impact on LGU

3. Rationale and Objectives

            State reason / purpose of the project (300 words or ½ page)

Describe the situation before the project, main problems, consequences, attempted solutions

Present project purpose and priorities, i.e. satisfying basic needs

Present project objectives

4. Project History (150 words or ¼ page)

Describe how project was initiated

Identify champions of the project in the LGU

Explain process of setting goals and determining strategies and methodologies (how, by whom)

Indicate period project was implemented

Project Results

      Highlight main accomplishments (300 words or ½ page, use photos to show results)

Describe extend project purpose / objectives were met

Outline indicators used to measure results

Identify project effects in key areas

Include participant’s comment re project impact

Key Implementation Steps (up to 4 pages use photos to show steps)

Present main implementation stage or milestone in chronological order

Each key stage / step should be detailed with related sub steps
